CPMT Society Chapter Subsidy Program

Program Purpose: The purpose of the CPMT Chapter Subsidy Program is to promote and support chapter membership and program activities. Maximum Funding: Maximum 1999 funding for the program is $10,000. Each CPMT Society Chapter can apply for a maximum subsidy of $500.00 in 1999.
Criteria: A written proposal for the chapter membership or program activity must be submitted. The proposal must contain the following information:
Activity Title Contact name, address, phone number, fax and email Proposal Membership or Program Benefits Budget Other sources of funding Volunteer involvement Approval by Chapter Chair and Treasurer with their signatures. Deadline: Projects will be evaluated on individual merit on a first-come first-served basis until project funding is exceeded. The project must be completed in 1999 and a follow up report must be submitted by 15 January 2000.
