Program Purpose: The purpose of the CPMT Chapter Subsidy Program is to promote and support chapter membership and program activities.

Maximum Funding: Maximum 1999 funding for the program is $10,000. Each CPMT Society Chapter can apply for a maximum subsidy of $500.00 in 1999.

Criteria: A written proposal for the chapter membership or program activity must be submitted. The proposal must contain the following information:

  • Activity Title
  • Contact name, address, phone number, fax and email
  • Proposal
  • Membership or Program Benefits
  • Budget
  • Other sources of funding
  • Volunteer involvement
  • Approval by Chapter Chair and Treasurer with their signatures. If the Chapter doesn't have a Treasurer, a second Chapter member must also sign the proposal.

Deadline: Projects will be evaluated on individual merit on a first-come first-served basis until project funding is exceeded. The project must be completed in 1999 and a follow up report must be submitted by 15 January 2000.
